1. Please complete this form to request a Carers Assessment.
  
  A carer is defined under the Social Services and Wellbeing Act 2014, as ‘A person who provides or intends to provide care for an adult or disabled child’
  
  **Please, only use this referral to request a carer’s assessment. Please use the “Contact Assist Online” to make all other social care enquiries.
  
  If the enquiry is urgent and you require an immediate response, please contact:
  Assist 0345 602 7050 (8.30 - 4.45 Monday - Thursday and 8.30 - 4.15 Friday)
  Emergency Duty Team 0845 0544 847 (out of hours service)
