All Wales Dementia Care Pathway of Standards
The All Wales Dementia Care Pathway of Standards were launched by Welsh Government in March 2021.
In November 2021 Powys held a conference to introduce the Standards locally to identify people from Statutory and Third sector organisations who were interested in being involved in progressing alignment to the Standards in Powys.
Following the launch five task groups were developed:
1.Community Engagement
This standard is about community engagement using one locality within a region working in partnership, taking 6-12 months to engage with that community to learn, evidence, and analyse what people and agencies have identified it needs e.g. Identify 'what dementia care and intervention looks like around here'. This will produce a vision and growth (delivery) plan. The chosen community in Powys is Rhayader and events have taken in the community to identify what dementia care looks like.
2. Memory Assessment/Learning Disabilities/Mild Cognitive Impairment and Dementia Connector
This standard includes improvement in diagnostic rates, services available to people affected by dementia including education, learning disabilities service provision, those diagnosed with Mild Cognitive Impairment and the proposed Dementia Connector role.
3. Dementia Friendly Hospital Charter
Wales will adopt the Dementia Friendly Hospital Charter with a regular review of implementation and outcomes.
4.Learning and Organisational Development
All staff delivering care at all levels within all disciplines and settings, will have the opportunity to participate in person centred learning and development with support to implement into daily practice.
5.Measurement
Working in partnership, the region will deliver on the requirements of the agreed data items for reporting and assurance. 2022/23 was the readiness year allowing areas to prepare to implement the Standards during 2023/24. The groups have identified what projects they need to adopt to align to the Standards and several improvement projects have been identified.
