Early Help and Prevention at Home

The Home Support services, and project are part of a wider programme of work to help ensure that prevention and early help and support at home is consistently available for adults across Powys.

The Home Support Service

Home Support is an early help and prevention service for citizens (50+) that enables and provides the support and practical assistance an individual may need in their day-to-day life to live at home with confidence, in good health, independently and safely.

Home support is all about putting an individual in control of their care and support, so they can live at home as independently as possible. The Home Support teams work closely with individuals, their family and anyone else in their life, to help them with the support that they need. They also work alongside other organisations in the local community to help provide better all-round support. Home Support services are free. Access is not means tested or dependent on inclusion/exclusion criteria and individuals can self-refer. Once an individual becomes a member of Home Support they can dip in and out of the service according to their needs without having to be re-referred. Some of the things Home Support can help with include:

  • Care line responders 24/7/365
  • Proactive wellbeing checks
  • Promotion of independence and wellbeing and healthy lifestyles
  • Practical support and assistance (help with appointments, accessing prescriptions or essential shopping)
  • Assist carers with their role 
  • Temporary personal support and care
  • Signposting, referral and help to access local community groups and supportive networks (including technology enabled care (TEC)) and services

Support at Home across Powys 

The Home Support project was established in 2018. The aim was to test out the impact of existing the Home Support service in Rhayader by extending this to Llandrindod Wells (as one locality) and establishing two new services in the East Radnor (Presteigne and Knighton), and Llanidloes localities. The vision was to set up Home Support services in all thirteen localities in Powys.

The commitment to continuous learning and improvement has been integral to the Home Support project from the outset. This has involved two key activities. The first involves recording and reporting all service activities (quantitative data) in relation to the service outcomes. This helps to show who is being supported, what has been delivered, and the difference the service has made. The second activity helps show how well the service has been delivered/received and includes feedback from service members and their carers (annual survey, compliments, complaints, and case studies). This ongoing evaluation has served to secure continued project and mainstream funding as well as provide extensive local evidence demonstrating the impact that Home Support services have made to individuals lives. In July 2021, the Powys County Council Cabinet approved the proposal to work with local services and communities to roll out and mainstream support at home services from the original three localities in Powys, to all thirteen. In October 2022, the fourth Home Support service was established in the Builth and Llanwrtyd Wells locality, and plans to continue to establish Home Support in the remaining nine localities are underway, via the Powys Prevention, Early Help and Support @ Home Collaborative (The Collaborative), which was set up in November 2023. The aim is to bring partners across proven prevention and early support initiatives together. The Collaborative will steer, advise, support and secure innovative, sustainable, and consistent models of prevention, early help, and support at home in all thirteen localities in Powys. In doing this, The Collaborative will help realise the Home Support vision for sustainable support at home services across Powys as part of a wider early help and prevention at home 'offer.'
